How to apply for maternity leave

When you notify your line manager that you are pregnant, the first step is to undertake the necessary risk assessments (PDF, 1MB) with your manager. Please note that there is also a COVID risk assessment to undertake.

You also need to complete the application for maternity leave form (MS Word, 34KB) and send it to the HR Helpdesk with your MATB1 so that maternity pay and entitlements are processed. Once we receive this, we will send you a letter confirming the dates and entitlement. You usually receive your MATB1 form from your midwife at your 20-week appointment.
